Patients are seen by appointment only. To schedule an appointment, please contact our east office, 316-684-2838 since all appointments are made through this office.

Our satellite offices are located in Winfield, Chanute, Newton, Great Bend and El Dorado. Please call 800-794-4368 when scheduling an appointment for these locations.

New Patients
When you come in for your first visit, you will need to complete these forms regarding your medical history and insurance information. You can save time by downloading these forms now and completing them at home. You may fax these forms to our New Patient Information Dept. at 316-684-3326 along with a copy of the front and back of your insurance card prior to your appointment. If you don't have access to a fax machine, please remember to bring in the completed forms when you come for your visit.

Emergency Calls
If you find it necessary to contact the doctor after regular office hours, the answering service will forward all messages to your doctor or the physician on call. Your call will be returned at the earliest opportunity.
