Appointments
To make an appointment, call either: 316-722-5811, 316-684-2838, or 800-794-4368, and ask for the Audio Department. We will be more than happy to schedule your appointment. You can also schedule your next appointment while you're in the office.

If you do not have an appointment and just walk in, there is a possibility that you can be seen — but you will have to wait until someone in the department has a free moment. The wait could be anywhere from 10 minutes or longer. All scheduled patients will be seen first. All walk-ins will incur a $3.00 charge. We recommend scheduling an appointment to avoid any waiting unless there is an emergency.

New Patients
When you come in for your first visit, you will need to complete these forms regarding your medical history and insurance information. You can save time by downloading these forms now and completing them at home. You may fax these forms to our New Patient Information Dept. at 316-684-3326 along with a copy of the front and back of your insurance card prior to your appointment. If you don't have access to a fax machine, please remember to bring in the completed forms when you come for your visit.
